This page is a valuable resource for patients seeking to make informed decisions about their treatment. At UroFill®, we believe that knowledge is the best defence against misinformation, and we are committed to providing you with all the information you need to confidently move forward.
The Anatomy of UroFill®: Your Essential Guide
The Key to Natural Looking Results
At UroFill®, we utilise a scientifically backed technique built on generations of urological advancements and surgical expertise.
Unlike other girth enhancement methods created by medical entrepreneurs and opportunistically adopted by non-specialists, UroFill® is urologist-founded, urologist-patented, and exclusively delivered by RACS-certified urologists.
Our mission is not driven by profit but by a commitment to safeguarding men’s health with the highest standard of care.
By establishing this gold standard, rooted in deep anatomical knowledge and extensive experience, we aim to prevent the need for corrective procedures caused by botched cosmetic penile surgeries performed by non-experts—because when complications arise, it’s us, the urologists, who ultimately correct them.

Critical Layers For Safe Enlargement
The penis is composed of several critical layers, and understanding these is essential for safe penile girth enhancement

Corpus Spongiosum
The tissue surrounding the urethra, ensuring a passageway for urine and semen.
Tunica Albuginea
The fibrous envelope that constrains the corpora cavernosa and maintains erection rigidity.
Corpora Cavernosa
The two cylinders responsible for erection by filling with blood.
Dartos and Buck’s Fascia
The layers of connective tissue where UroFill® is injected. This space between Buck’s fascia and the Dartos fascia is crucial for adding volume in a way that maintains both the look and function of a normal penis.

Why UroFill® Targets the Space Between Dartos and Buck’s Fascia
The space between Buck’s fascia and the dartos fascia is a critical anatomical zone for penile girth enhancement. UroFill® injects hyaluronic acid (HA) into this specific layer to:
Maintain Natural Appearance – By working within the correct fascial plane, UroFill® ensures that the added girth blends naturally with the existing penile structure, avoiding the unnatural "inflated" look that often results from improper injection techniques.
Preserve Function – Injecting into this space prevents interference with the erectile function provided by the corpora cavernosa. It also helps avoid damage to essential nerves and blood vessels, reducing the risk of complications like erectile dysfunction.
Improve Girth Retention – The precise placement of HA in this avascular space slows its natural breakdown by the body, ensuring that the girth enhancement lasts longer compared to other techniques that inject in suboptimal layers.

Patented Precision

When it comes to increasing thickness, precision, proper technique and understanding of the anatomy are crucial for achieving aesthetically pleasing results.
Unfortunately, non-urologists who perform girth augmentation procedures claim to inject filler into the correct anatomical planes but typically miss the mark, leading to suboptimal outcomes.
Here’s why non-UroFill® providers often inject subdermally, and why this approach generates inferior results compared to UroFill®'s precise, urologist-performed technique:
The Importance of Expertise in Penile Surgery – Dr. Ross Calopedos, a FRACS urologist with extensive experience in complex penile surgeries such as penile disassembly, Penile Implants, Peyronie’s Disease Correction, and Penile Fracture Repair, understands a critical anatomical truth: Buck’s and Dartos Fascia are fused, not free-floating as often depicted in medical illustrations. The loose, free-moving tissue is the subdermal areolar tissue, and the correct anatomical plane must be carefully created for optimal results.
Patented for Your Protection - UroFill® is specifically designed to develop the correct anatomical space, ensuring your safety and delivering exceptional, natural-looking outcomes.
Why Non-Urologists Fall Short – Non-urologists, lacking this level of anatomical expertise, often inject fillers sub-dermal (just beneath the skin) rather than in the proper plane. This mistake not only compromises the cosmetic outcome but significantly increases the risk of complications.
Lack of Precision in Non-UroFill® Techniques: Non-urologist providers often use blunt-tipped cannulas with "in-and-out" or "fanning" motions, which can cause extensive damage to the delicate fascial layers.
Risk of Bloated & Deformed Appearance: Poor technique from these non-specialists, coupled with large injection volumes, leads to deformed or bloated appearance due to improper filler placement.
Soft Erections: Hyaluronic acid (HA) injected subdermally cause a soft consistency of the tissue that is particularly noticeable during erection, detracting from the normal feel and aesthetic.
Urologist Expertise is Key: Urologists trained in penile surgery know how to navigate the narrow window for HA injection with precision, ensuring natural-looking, lasting results.
Risks of Subdermal Injection of Filler in the Penis

Filler injected subdermally exposes the product to a dense network of vessels in the subdermal vascular plexus. This has several negative consequences:
Higher Resorption Rates: The body's natural enzyme, hyaluronidase, which breaks down hyaluronic acid (HA), is present in the superficial microcirculation. When HA is placed too superficially, the increased exposure to this enzyme accelerates the resorption process, leading to faster breakdown and less durable results. Hyaluronic acid is expensive, don't waste it!
Increased Risk of Embolic Events: Injecting near the large superficial veins, particularly with a cannula, greatly heightens the risk of embolic complications, which can lead to severe outcomes, including death.
Unnatural Appearance: Large volumes of filler in the subdermal space often create an inflated, unnatural look, as the product doesn’t conform to the penis’s natural anatomical structure.
Risk of Skin Necrosis, Scarring and Deformity: When the subdermal space is overfilled, the blood supply to the skin is compromised. This can lead skin death or firm areas where scar formation has occurred. These problems are not only unsightly but also reduce the flexibility of the penile skin, which limits expansion during erection and causes angulation deformities. Correcting this requires removing the affected tissue and covering it with a skin graft.

The Dangers of Injecting Too Deep, Another Reason to Choose UroFill®
Another key difference between UroFill® and providers who are not urologists is another critical fact about penis anatomy: Buck's fascia (the floor of the correct anatomical space) is firmly attached to the tunica albuginea, which surrounds the erectile tissues.
This important detail is only fully understood by urologists who specialise in penile surgery. It becomes clear when correcting complex Peyronie’s disease deformities, where surgical elevation of the vital nerves and blood vessels off the erectile bodies is necessary. Urologists, with this deep anatomical knowledge, can safely and precisely place HA in the right space.
Injecting HA too deep, particularly into the erectile bodies, can cause long-term damage and result in serious complications.

Key Risks of Deep Injections:
Erectile Dysfunction (ED): When filler is injected too deeply into the corpora cavernosa, it can severely damage the vital erectile tissue and block crucial blood vessels. This can lead to permanent erectile dysfunction (ED). While non-specialist providers may claim that their "atraumatic" cannulas prevent harm, this is often not the case. Unfortunately, we have seen multiple men who suffered this exact complication after receiving treatment from non-urologists. Despite efforts to dissolve the misplaced HA, the damage was irreversible, and all these patients required penile implant surgery to restore their sexual function.
Compromised Tunica Albuginea: The tunica albuginea is the tough, protective layer surrounding the erectile tissue. It measures approximately 2mm thick when the penis is flaccid but thins to just 0.25mm during an erection. Any procedure that involves injecting HA or other material during an artificially induced erection (such as through intracavernosal injections—often referred to as ICI, Caverject, Bimix, Trimix, Alprostadil or Papaverine) dramatically increases the risk of severe complications. These risks are especially high when performed by non-urologists, despite any claims of expertise.

The Benefit of Urological Expertise
Urologists trained in penile surgery possess a detailed understanding of the anatomy required to safely create the correct injection planes. They have the specialised knowledge to navigate between Buck’s fascia, Dartos fascia, and the tunica albuginea, ensuring that the material is not introduced too deeply or superficially. Precision is key—just nanometers separate successful outcomes from disaster.
With over 6,000 satisfied UroFill® patients and more than 200,000 injection treatments performed, we have not encountered a single case of this type of complication. Beware of flashy marketing that prioritizes "aesthetics" over function. Many unfortunate stories have unfolded due to a lack of transparency from providers offering quick-fix solutions. Always demand the best—your sexual health and long-term satisfaction depend on it.
